At PrimeCare Medical Clinic we know that providing good care is not just delivering quality medical care. It involves convenience and accessibility. We want your visit to be as pleasant as possible. For your convenience, we have provided you the opportunity to register prior to your first visit with us. You can do this by simply following the instructions below before you come into the clinic.

Print a copy of the three documents included in the following Adobe Reader  Package (PDS). To do this, Click on the PDF Document and then click on the print button.  Once you have printed it, you can close the document.   The three documents that are included, are listed below.

• PrimeCare Medical Clinic Notice of Privacy Practice  (Keep this copy for your records.)
• Acknowledgement of Receipt of Notice for Privacy Practice (This form will need to be filled out and signed by you.)
• New Patient Registration Form (This form will need to be filled out and signed by you.)

Pre-Registration Notice and Forms

When you come in for your first visit, bring a completed copy of the second and third Document.   That’s it!
