What should I bring to my child’s first appointment?

What age range of children do you treat?
We care for children from birth through age 18. After that, they can continue receiving care with us; however, if they were seeing one of our pediatricians, they will transition to a family medicine provider.
Do you provide sports physicals or school forms?
Yes, we offer sports physicals and can complete school health forms during your child’s visit. Just bring any required paperwork with you when you come in!
Do you offer routine well-child visits and immunizations?
Yes, we provide routine well-child checkups and all recommended childhood immunizations. These visits are important for tracking your child’s growth, development, and overall health.
